You wear a gown and they uncover and cover each area as they examine, you are never totally naked. They will look between you toes on your genitals-- everywhere, I would not think you could get skin cancer on your genitals especially if you don't go skinny dipping but it does happen. I think this is if you go for a full body scan not just a regular screening.
i would imagine in a gown, and he/she wouldn't check anything covered by your underwear, unless you have a concern. what are you going for?
